Job Title: Family Nurse Practitioner

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Paragon Healthcare. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the overall success of our organization.

Job Summary:

The Family Nurse Practitioner will be responsible for providing primary and specialty care to patients in a clinical setting. This includes conducting physical assessments, developing and implementing treatment plans, and educating patients and their families on health and wellness.

Key Responsibilities:
  • Administer ordered therapies according to prescriber order, facility protocols, and company policy and procedures.
  • Complete admission process as directed by Center Director.
  • Perform physical assessment on each patient as warranted and for new consults as warranted by the patient diagnosis.
  • Document all pertinent data in the patient's medical record.
  • Report all significant changes or observations to the Center Director and be responsible for the follow-up on any problem identified.
  • Communicate effectively with other members of the IV team on patient status and patient confidentiality at all times by abiding by HIPAA laws.
  • Educate patients and/or family members regarding therapy plan.
  • Participate in the Quality Improvement program.
  • Ensure that all medication orders are complete, accurate, and up-to-date prior to administration.
Requirements:
  • Requires an MS in Nursing and a minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
  • Experience with IVs required.
  • Current, active, and valid unrestricted NP license to practice as a healthcare professional with scope of license in applicable state required.
  • Active, current, and valid Family Nurse Practitioner licensure is required if this individual is providing services in multiple states.
Preferred Skills:
  • Healthcare experience with IV's strongly preferred.
  • 2+ years of experience with IV preferred.
  • Port, PICC & Peripheral Line experienced preferred.
  • Medication Mixing experience preferred.
  • Titration experience preferred.
