Job Summary

We are seeking a compassionate Medical Care Professional to join our team at Abode Care Partners.

About the Role

This position involves providing evidence-based medical care to seniors and patients with complex needs in various settings. The successful candidate will have the opportunity to grow their medical skills while working collaboratively with a team of healthcare professionals.

Key Responsibilities:
  • Diagnosing and Treating Illnesses: Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Maintaining Comprehensive Patient Care Plans: Order and interpret laboratory and diagnostic testing as needed, collaborate with caregivers and other healthcare professionals in providing interdisciplinary care.
  • Providing Referrals: Refer patients to other healthcare practitioners when necessary and ensure patient medical records are kept up to date.
Requirements:
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, Active state DEA licensure, and Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, excellent time management skills with accountability for practitioner's daily workflow, proficiency in medical instruments and equipment, and strong knowledge base of HIPAA and OSHA regulations.

Estimated Salary Range:

$100,000.00 - $120,000.00 per year.
