About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Palliative Care team at Illumia. As a Palliative Care Nurse Specialist, you will play a vital role in providing high-quality end-of-life care to our patients and their families.

Key Responsibilities
  • Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Provide education and support to patients and families on disease management, self-care techniques, and end-of-life care.
  • Document timely clinical records and maintain accurate patient information.
  • Participate in the development and implementation of palliative care plans, ensuring individualized care for each patient.
  • Facilitate communication between patients, families, and healthcare professionals to ensure comprehensive care.
Requirements
  • 2 years of prior skilled Registered Nurse experience, preferably in palliative care, oncology, hospice, or geriatric care.
  • Demonstrated ability to provide quality patient care and excellent communication skills.
  • Current RN license to practice in the state and CPR certification.
  • Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel.
