We are seeking a skilled Travel Nurse Case Manager RN to join our team at Eden Medical Center. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions throughout the acute care experience.
Key Responsibilities- Care Coordination: Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to develop and implement effective care plans.
- Resource Coordination: Facilitate care across the continuum by coordinating with patients, families, physicians, and the interdisciplinary team.
- Quality Improvement: Monitor length of stay (LOS) and outliers, and participate in patient rounds to ensure optimal health outcomes.
- Documentation: Accurately document patient information in the EHR and other electronic software.
- Regulatory Compliance: Maintain knowledge of CMS and Joint Commission Transitions of Care requirements.
- Current CA RN License
- BLS Certification
- Ability to work in a fast-paced environment
- Strong communication and interpersonal skills
- Knowledge of case management principles and practices
Please note that certifications such as ACLS, PALS, NRP, CCRN, TNCC, EFM, and State License may have special instructions or deadlines.
