Position Overview

Vertis Therapy is expanding and is seeking a dedicated PRN Speech Language Pathologist to provide consistent care at our Assisted Living facilities. As a therapist-owned organization, we prioritize delivering exceptional, patient-focused services.

Key Responsibilities

  • Assess and evaluate patient information to inform treatment strategies.
  • Conduct comprehensive functional assessments using standardized methods, addressing home and vocational needs.
  • Create and execute personalized treatment plans tailored to individual patient requirements, ensuring realistic and achievable goals.
  • Adhere to established guidelines for evaluation, treatment, and documentation.
  • Maintain accurate records of patient progress at least once every week.
  • Adjust treatment plans as necessary throughout the care process.
  • Document treatment courses clearly and concisely, including discharge summaries in the required format.
  • Educate patients about program services and treatment methods relevant to their needs, including potential risks.
  • Collaborate with nursing staff, patients, and families to enhance the patient's functional capabilities.
  • Guide patients and their families in exercises and therapeutic practices to continue post-discharge.
  • Assist in referrals for additional services as needed, while staying informed about community resources.
  • Participate in rehabilitation team meetings to discuss patient progress and needs.
  • Engage in departmental meetings and training sessions as required.
  • Pursue continuing education opportunities to maintain clinical proficiency.
  • Contribute to quality improvement initiatives by responding to audits and satisfaction surveys.
  • Comply with Vertis Therapy policies and regulatory standards.
  • Advocate for Vertis Therapy’s programs and services through community interactions.
  • Perform additional duties as assigned.

Qualifications

  • Master’s degree in Speech/Language Pathology.
  • Valid license/certification in the state of practice.
  • Preferred experience of at least one year in geriatric rehabilitation.
  • Must fulfill educational requirements for certification and be accumulating supervised experience or meet the criteria for a Certificate of Clinical Competence in speech-language pathology from the American Speech-Language Hearing Association.
  • Strong oral and written communication skills.
  • Proficient computer skills for documentation and job responsibilities.
  • Proven ability to apply principles and techniques in speech therapy effectively.
  • Knowledge of accreditation standards and compliance requirements.
