Job Summary:

Richmond Community Services is seeking a skilled Nurse Practitioner to join our team. The ideal candidate will have experience working with individuals with developmental disabilities and a strong background in family practice, pediatrics, or rehab nursing.

Key Responsibilities:

  • Develop and implement medical care plans for individuals served in conjunction with the Individual Treatment Team (ITT) & Individual Program Plan (IPP).
  • Perform physical exams, annually and as needed, including gross examination of vision and hearing and prepares and documents findings of annual physical exams.
  • Write medical orders, prescriptions for medications, devices, and immunizing agents in accordance with standard medical practice and clinical protocols.
  • Evaluate, initiate and monitor in conjunction with rehabilitation staff, individualized habilitation programs.
  • Document and sign all pertinent information in the progress notes and reviews all pertinent progress notes.
  • Communicate with other health care professionals at acute care facilities or clinics to discuss individual's medical status.
  • Conduct initial history and physical exam of new admission with documentation of findings and recommendations.
  • As needed, communicate to Team Leaders any medical information or changes in medical status of individuals served.
  • Reviews pertinent history and conducts assessments, as necessary, of individuals served.
  • Request and evaluate diagnostic testing, including all laboratory screening and special studies, as indicated by the medical status of individuals served.
  • Request all needed consultations and reviews all recommendations by consultants. Writes orders based upon consultant recommendations, standard medical practice and clinical protocols.
  • Makes diagnosis and develops appropriate treatment plans as per standard Nurse Practitioner protocols.
  • Monitors prescribed seizure control medications of individuals including blood serum levels when appropriate
  • Conducts and documents discharge physical examination and prepares discharge review, including written recommendations to new placement.

Requirements:

  • Graduate of an accredited professional school of Nursing with a concentration in Family Practice, Pediatrics or Rehab Nursing preferred.
  • Current NY State RNNP license and certification through ANCC or AANP.
  • Must have a current license, CPR certification, Infection Control certificate, and DEA certificate.
  • A minimum of one year of experience working with individuals with developmental disabilities.
  • Proficiency with EMR and e-prescribing programs.
