About This Role

This exciting opportunity is for a Clinical Healthcare Specialist, RN, to work closely with our Inpatient Care Management team. As a key member of this team, you will play a vital role in ensuring seamless transitions between care settings, facilitating timely discharges, and promoting high-quality patient outcomes.

Your primary responsibilities will include assessing patient needs, conducting concurrent reviews, implementing effective discharge plans, and communicating with internal and external stakeholders to ensure optimal patient care.

In addition to your clinical expertise, strong communication and problem-solving skills are essential for success in this role. You will work collaboratively with healthcare providers, patients, and families to achieve the best possible outcomes.

Key Responsibilities
  • Conduct thorough assessments of patient needs and develop personalized care plans to address their unique requirements.
  • Collaborate with healthcare providers and teams to ensure seamless transitions between care settings, reducing readmissions and improving patient satisfaction.
  • Utilize clinical knowledge and expertise to evaluate requests for authorization, interpreting medical necessity criteria and ensuring alignment with organizational policies.
  • Communicate effectively with patients, families, and caregivers to educate them about treatment options, medication management, and post-discharge care.
  • Participate in training and development programs to enhance your clinical skills and stay up-to-date with best practices in healthcare.
Requirements
  • Bachelor's degree in Nursing (BSN) or higher; current California RN license required.
  • A minimum of 3 years' experience in acute care settings, with a proven track record of successful case management and transition coordination.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with healthcare providers, patients, and families.
  • Basic computer skills, including electronic medical records, Outlook email, and Microsoft Office suite.
  • Familiarity with managed care concepts, Medicare/Medi-Cal guidelines, and InterQual criteria is highly desirable.
Salary and Benefits

The anticipated salary range for this position is $85,000 - $110,000 per annum, commensurate with experience and qualifications.
