We are seeking a skilled Social Worker to join our Home Health team on a part-time basis. As a Social Worker, you will provide essential social work services to individuals and groups, including screening, psychosocial assessment, discharge planning, and more.

Key Responsibilities
  • Conduct initial assessments and establish plans of care through psychosocial, financial, functional, and environmental data
  • Provide ongoing assessment of patient/family needs and apply appropriate interventions
  • Update and revise plans of care as indicated
  • Plan for discharge from medical social services according to organization standards
  • Implement interventions substantiated by assessment and collaborate with community agencies and institutions
  • Document activity according to department and program protocols or standards
Requirements
  • Master's Degree in Social Work
  • 2 years of experience in social work, including experience in a home care or hospice environment
  • Exceptional assessment skills, understanding of family dynamics, and psychosocial issues related to disability and illness
  • Ability to provide short-term counseling to patients and their families
  • Exceptional collaboration skills, advocacy abilities, and ability to perform as a team player
Working Conditions

This role is subject to exposure to chemical and hazardous waste and communicable disease, requiring protective clothing to be worn as necessary.
