Job Summary

This is a unique opportunity to work as a Physical Therapist in a dynamic and innovative healthcare organization. As a member of our interdisciplinary care team, you will be responsible for delivering personalized rehabilitation therapy to patients in their homes or care facilities.

Key Responsibilities:

  • Conduct comprehensive assessments of patients to determine their needs and develop effective treatment plans.
  • Deliver high-quality rehabilitation therapy to patients, adapting to their evolving needs and responding with plan of care adjustments.
  • Practice independently in the community, collaborating with other healthcare professionals to ensure seamless care.

Requirements:

  • Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside the borough of Manhattan.
