Key Responsibilities- Deliver Quality Patient Care: Provide compassionate and individualized care to patients, ensuring their physical, emotional, and social needs are met.
- Coordinate Care: Work closely with patients, families, and healthcare teams to develop and implement personalized care plans.
- Assess and Document: Continuously assess patient condition and document all activities, assessments, and care provided.
- Participate in Care Planning: Collaborate with healthcare teams to develop and update patient care plans, ensuring effective and efficient care delivery.
- Provide Education and Support: Educate patients and families on disease management, self-care techniques, and prevention strategies.
- Education: Graduate of an accredited school of nursing.
- Licensure: Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Certification: Current CPR certification.
- Experience: Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home Health Experience: Previous experience in home health care settings.
- Skills and Abilities: Strong attention to detail, effective time management, and excellent problem-solving and communication skills.
- Physical Demands: Ability to lift 50 pounds, stand for prolonged periods, and move equipment without assistance.
- Visual and Hearing Acuity: Strong visual and hearing acuity, with ability to respond to emergency situations.
