Job Title: Licensed Practical Nurse

Boswell Transitional Care of Cascadia is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality care to our residents in Sun City, Arizona.

Job Summary:

Under the supervision of a Registered Nurse (RN), the LPN will provide person-centered nursing care to elderly, acutely ill, or injured residents. The ideal candidate will possess excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Key Responsibilities:
  • Provide direct care and assist with care as directed and consistent with scope of practice and competency.
  • Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
  • Make rounds with physicians and other team members as needed.
  • Participate in resident evaluation and care planning activities, reviewing and revising residents' assessments and care plans as needed.
  • Coordinate residents' care activities and communicate changes to other team members and resident family/significant other.
  • Administer medications, transcribe, communicate, and implement physician orders, obtaining RN signature as required by state regulations or company policy.
  • Communicate with resident's physician and assist with emergencies.
  • Monitor completeness and accuracy of own clinical medical record entries and those of assigned LPN and Nurse Aides daily.
Requirements:
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.
  • Current certification in one of the following clinical specific areas: Long-term care, Gerontology, Wound care, IV therapy, Ventilator, Pulmonary & Tracheostomy, or Hospice & Palliative care.
  • Six months experience in a long-term care environment preferred.
