Job Summary:

The Physical Therapist will be responsible for providing high-quality physical therapy services to patients in Area #12 San Jose and Area #11 (Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Morgan Hill, Mt. Hamilton, Saratoga, Los Gatos, San Martin).

Responsibilities:
  • Provide physical therapy services to patients in the assigned areas.
  • Conduct evaluations, assessments, and treatments to patients with various physical therapy needs.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care for patients.
  • Document patient progress and maintain accurate records.
Requirements:
  • Current CA PT License.
  • BLS certification.
  • Drivers License and auto insurance.
  • Annual TB test, MMR, Varicella, HepB, TDap (10 years), and Flu vaccination.
  • Drug screen and physical examination.
Work Environment:

The Physical Therapist will work in a fast-paced environment, providing services to patients in various settings, including clinics and patient homes.
