LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Grenada, Mississippi.
Job SummaryWe are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions to improve patient outcomes.
Responsibilities- Evaluate patient functional status and physical therapy needs, and assist the physician in developing the therapy plan of care.
- Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
