Ambercare – an Addus family company is seeking a skilled Home Health Nurse to join our team in the city of Espanola. This is an excellent opportunity for a dedicated and compassionate nurse to provide exceptional care to patients in need.

Compensation: Earn up to $40/hourly

About the Role:

We are looking for a highly motivated and experienced Registered Nurse to assume responsibility for triage of after-hours patient and family phone calls. You will determine the need for services including assessments, symptom management, and care plan updates, delegating nursing visits to appropriate staff members as needed.

You will obtain data on physical, psychological, social, and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team when necessary. Additionally, you will communicate with the attending physician, other interdisciplinary staff members, and other agencies to coordinate optimal care.

Key Responsibilities:

  • Clinical Judgment: Assume responsibility for triage of after-hours patient and family phone calls and make clinical decisions regarding care plans
  • Communication: Develop and maintain effective relationships with patients, families, and healthcare teams
  • Collaboration: Work collaboratively with interdisciplinary teams to coordinate patient care
  • Documentation: Maintain accurate clinical records through timely preparation of care plans and clinical notes

Requirements:

  • Bachelor's degree in Nursing from an accredited program
  • Current license to practice as a Registered Nurse in the state
  • Minimum 2 years of varied work experience as a professional nurse
  • Demonstrated skill in nursing practices and clinical judgments
  • Ability to cope with family emotional stress and tolerance of individual lifestyles
  • Current CPR Certification
  • Valid driver's license and proof of insurance
