Hospice Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager in Hospice Home Care. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Administer medication and treatment as prescribed by the physician, while also providing emotional and spiritual support to patients and their families.
- Document patient progress and maintain accurate clinical notes to ensure continuity of care.
- Participate in interdisciplinary team meetings to discuss patient care and address any concerns or issues.
- Graduate of an accredited nursing school with current licensure to practice in the state of Arkansas.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
