We are seeking a Registered Nurse, Care Navigation to join our team in Evanston, IL or New York, NY.

The successful candidate will be responsible for driving our initiatives to identify opportunities to help members access high-quality healthcare.

Key Responsibilities:

  • Review medical charts to identify cost-savings opportunities for members
  • Leverage data to find high-quality treatment options
  • Support members throughout their health journey, including answering patient inquiries, making referrals, and conducting follow-up activities

Requirements:

  • Active Registered Nurse license in the state of Illinois or New York or eligible for RN licensure in Illinois / New York
  • Bachelor's or Associate's degree in Nursing, or equivalent experience
  • Extensive knowledge of medical terminology, diagnosis codes, and healthcare delivery processes
  • Ability to multi-task effectively and work in a fast-paced, dynamic environment
  • Demonstrated proficiency in computer skills, including data entry, documentation, and internet searches
