We are seeking a skilled Home Health Physical Therapist to join our team at CenterWell Home Health.
As a member of our care team, you will work closely with patients to develop and implement personalized rehabilitation plans, helping them to regain their independence and improve their overall quality of life.
Key Responsibilities- Plan and administer physical therapy treatment and training to patients with various injuries, illnesses, and functional disabilities.
- Conduct thorough assessments to identify patients' physical strengths and limitations, and develop targeted treatment plans to address their needs.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care and effective communication.
- Provide education and training to patients and their families on proper care and management techniques.
- Document patient progress and outcomes, and participate in regular team meetings to discuss patient care and treatment plans.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
