Job Title: Primary Care Physician

We're seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a Primary Care Physician, you will play a vital role in providing high-quality, patient-centered care to our diverse patient population.

Key Responsibilities:
  • Assess and manage acute and non-acute clinical problems in patients of all ages
  • Develop and implement individualized patient management plans, taking into account the patient's cultural background, level of understanding, and support systems
  • Provide education and counseling to patients and their families on health promotion, disease prevention, and management of chronic conditions
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Current, active MD licensure in the state of employment
  • Board Eligibility or Certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Minimum of 2 years' clinical experience in geriatric, adult, or family practice setting
  • Minimum of 2 years' clinical leadership experience preferred
