Nurse Practitioner Job Description

We are seeking a skilled Nurse Practitioner to join our team at HealthCare Support. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Travel to Pierce, Kitsap, Thurston, and Lewis counties to perform comprehensive patient assessments, addressing both medical and behavioral conditions.
  • Manage acute and chronic illnesses proficiently, ensuring accurate and thorough documentation, including ICD 10 conditions.
  • Communicate and collaborate with the interdisciplinary care team, conducting advanced illness and care planning conversations, while providing counseling and education to patients and caregivers.
Requirements:
  • Active and unrestricted license in Washington.
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
Preferred Qualifications:
  • 1+ year of clinical experience in practice (long-term care setting preferred).
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end of life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.

Pay Details:

$80.00-$95.00 an hour.
