Wraparound Facilitator

The Wraparound Facilitator provides high-fidelity wraparound services for youth and families, facilitating family choice and voice, and collaborating with key stakeholders, community partners, and team members.

• Ensure positive client experience and best practices by facilitating high-fidelity wraparound, staying updated on policies and procedures, and implementing feedback from site coaches and supervisors.
• Develop an organizational system to manage demands of DMHA requirements, System requirements, and additional funding sources.
• Ensure documentation is completed timely, accurately, and submitted as required, maintaining ethical standards of the System and funding sources.
• Establish positive working relationships with community groups, funders, partners, courts, political leaders, and other organizations to help achieve the organization's goals and ensure stakeholders are aware of programming and referrals.
• Manage funding sources and approved services, ensuring assessments are completed timely, services are approved, and information is relayed to external and internal providers.
• Assess risk and develop safety plans, report abuse or neglect, and assist in direct admission for acute care, PRTF/RTF placement, and SOF. Collaborate with courts and legal entities to ensure safety.
• Meet metrics assigned to this position, including compliance with internal and external trainings, documentation, and timely completion of tasks.

• Bachelor's Degree required.
• 2 years of experience with SED population or received clinical supervision by licensed provider.
• Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check.
• Completion of New Hire Orientation and all training requirements, including Relias.
• Current driver's license, acceptable driving record, and current auto insurance.
