Job Description

The Direct Support Professional is responsible for working with individuals to help them achieve their personal goals and create a fulfilling life.

Key Responsibilities:

  • Supervise assigned individuals according to the Person Served Supervision Policy #300.
  • Implement written treatment plans and record data accurately.
  • Perform life skills training to promote independence.
  • Complete required documentation within designated time frames.

Working Conditions:

The Direct Support Professional position requires the ability to exert up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects. The position involves working in indoor and outdoor environments, supporting individuals in the home and community, and frequent driving.

Requirements:

  • High school diploma or GED.
  • At least 21 years old (unless an exception has been made).
  • Successful completion of the TABE test.
  • Sufficient communication and writing skills to complete required documentation.
  • Ability to meet Department of Human Services Division and Developmental Disabilities and Goldie B. Floberg Center requirements to administer medications.
  • Successful background checks and no convictions of crimes that violate DHS or DCFS requirements.
  • Ability to work effectively as part of a team.
  • Meet physical demands described in the Working Conditions & Physical Demands section.

Essential Job Functions:

  • Ensure proper hygiene of all clients.
  • Drive and/or support clients on doctor appointments as needed.
  • Lead meaningful on-site activities and support clients in participating.
  • Memorize and accurately implement evacuation procedures.
  • Accurately and completely report on established written documentation systems.
