About the Job

We are seeking a skilled Physical Therapist to join our team at Supplemental Health Care. As a Physical Therapist, you will be responsible for providing high-quality patient care in a home health setting.

Job Summary

This is a travel assignment opportunity for a Physical Therapist to work with a partnering home health care provider in Alabama. The ideal candidate will have prior experience with OASIS and Medicare-certified visits.

Responsibilities
  • Provide physical therapy services to patients in their homes
  • Conduct thorough assessments and develop individualized treatment plans
  • Implement and modify treatment plans as needed to ensure optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes in accordance with regulatory requirements
Requirements
  • Graduate of an accredited Physical Therapy program
  • Licensed as a Physical Therapist in Alabama
  • CPR certification or ability to obtain
  • Previous experience with computer charting
  • 6 months of previous home health care experience
  • Valid U.S. driver's license and reliable transportation
