Job Description

We are seeking a skilled Physical Therapist Assistant to join our team. As a vital member of our interdisciplinary team, you will collaborate with patients, families, caregivers, and primary care providers to develop holistic plans of care focused on achieving physical therapy goals.

Your expertise will involve instructing, coaching, guiding, and intervening to support patients and families in achieving the best possible outcomes while remaining in their communities. You will promote optimal health, well-being, and quality of life for home health patients and their families by providing skilled, intermittent physical therapy in their homes.

As an independent practitioner, you will utilize evidence-based practice for assessment, clinical decision-making, clinical practice, and coordination of care across the continuum. Additionally, you will act as a case manager and team lead for patients requiring home health therapy services when nursing is not involved or physical therapy is the primary patient need.

Responsibilities:
  • Assess patients and collaborate with the interdisciplinary team and primary care provider to develop patient-centered plans of care.
  • Provide physical therapy treatment to patients/families/caregivers as ordered.
  • Complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to the primary care provider.
  • Supervise Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.

In this role, you will have the opportunity to work without direct supervision and provide care appropriate to the age of patients served. You will be required to maintain effective relationships with other healthcare professionals and demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.

Requirements

To be successful in this position, you should possess excellent customer service skills, knowledge of physical therapy assessment and treatment methods, and ability to maintain effective relationships with other healthcare professionals. Additionally, you should be able to develop skills in the use of technology used in home care settings.

The estimated salary for this position is $62,000 - $82,000 per year, depending on experience and qualifications. Benefits include competitive pay, opportunities for professional growth and development, and a supportive work environment.
