Advanced Practice Provider-Hospitalist- St. Nicholas

Prevea Health Services is seeking a skilled Advanced Practice Provider (APP) to join our Hospitalist team. As an APP, you will play a vital role in providing high-quality patient care and collaborating with our multidisciplinary team to ensure the best possible outcomes.
Responsibilities- Provide assistance to the Hospitalist service in gathering and validating daily patient data, such as vital signs, ancillary reports/results, intake/output reviews, and others.
- Perform patient rounds with the Hospitalist and independent rounds when appropriate and identify patients to be discharged.
- Ensure timely execution of Hospitalist orders.
- Offer input to patient care decisions and patient care planning.
- Act as a liaison between the physicians and the case management staff regarding issues on patient discharge planning needs.
- Assist in facilitating timely discharges of patients, which may include transcribing discharge orders, preparing prescriptions, and confirming discharge plans with family members/Case Management.
- Instruct patients/significant others and healthcare professionals regarding disease process, treatment plan, and admission through discharge needs.
- Utilize evidence-based medicine as basis for improvements in care and outcomes of Hospitalist patients.
- Willingness to learn new roles within scope of practice.
- Demonstrates ability to develop and maintain effective working relationships.
- Demonstrates organizational skills that allow for timely patient appointments and appropriate required follow-up.
- Presents a courteous and service-oriented image through positive and helpful interaction with patients and families, physicians, and co-workers.
The APP will work in a fast-paced hospital environment, collaborating with a multidisciplinary team to provide high-quality patient care. The ideal candidate will be able to work effectively in a team setting, communicate effectively with patients, families, and healthcare professionals, and maintain a high level of professionalism and integrity.
