Job Summary:
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Addus HomeCare. As an LPN, you will provide high-quality patient care and support to individuals in need of hospice services.
Key Responsibilities:
- Provide direct patient care as defined in State Nurse Practice Act
- Implement current nursing practice following a comprehensive assessment and the plan of care
- Provide accurate and timely documentation of patient services to reflect the plan of care
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
- Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
- Provide appropriate pain/symptom management. Evaluate patient's response to treatments/medications
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state
- Prior home health or hospice experience preferred with minimum of 1 year medical/surgical experience
- Maintains a current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
