**About the Role**
The Social Service Coordinator at St. John's Residence for Boys works closely with the social service and residential department staff to provide direct support to assigned resident youth and their families.
As a key member of the treatment team, the Case Support Specialist is responsible for implementing evidence-based models of care and developing goals that ensure safe, sound, and appropriate outcomes.
This full-time position involves working both on-site and in the community to secure services and achieve stated objectives.
**Key Responsibilities:**
- Work collaboratively with the treatment team to develop and execute individualized plans for resident youth
- Provide intensive support to assigned residents and their families to ensure successful outcomes
- Secure services and resources within the community to meet the needs of assigned residents
**Requirements:**
- Valid NYS driver's license (3-point maximum)
- Minimum BSW/BA degree in social work or related field
- Prior experience working with youth required; prior case management experience/knowledge of congregate settings a plus
