Job Title: Co-Therapist In-Home

Job Summary:

AltaPointe Health is seeking a highly skilled Co-Therapist In-Home to join our team. As a Co-Therapist In-Home, you will work closely with our program therapists to provide outreach therapy services to children and adolescents in their homes. This is a unique opportunity to make a meaningful difference in the lives of our consumers and their families.

Responsibilities:

  • Assess and reevaluate consumer needs utilizing the Strengths and Needs Assessment (CANS)
  • Provide linkage to community resources and arrange necessary transportation
  • Monitor consumers to ensure placement in the least restrictive environment
  • Provide ongoing monitoring of consumers' progress and needs
  • Support consumers in securing community resources and needed services
  • Collaborate with staff members and outside sources in the delivery and arrangement of case management services
  • Evaluate and assist in consumer discharge planning
  • Apply child/adolescent specific competencies
  • Deliver or arrange clinically necessary transportation
  • Act as co-therapist as needed, assisting program therapists in providing outreach therapy services
  • Provide crisis intervention and management
  • Seek clinical supervision and consultation as needed
  • Accept and employ suggestions for improvement
  • Enhance case management skills
  • Document in a timely fashion per AltaPointe policy
  • Document measurable and specific goals in the individualized service plan
  • Document types of consumer problems, treatment goals addressed, and services provided
  • Document case disposition, case closings, follow-up, and referrals
  • Treat consumers with care, dignity, and compassion
  • Respect consumers' privacy and confidentiality
  • Be pleasant and cooperative with others
  • Assist consumers and visitors as needed
  • Personal values do not inhibit ability to relate and care for others
  • Be sensitive to consumers' needs, expectations, and individual differences
  • Be gentle and calm to consumers and families
  • Effectively manage caseload based on consumer needs
  • Meet AltaPointe productivity standards
  • Actively participate in Performance Improvement activities
  • Actively participate in AltaPointe committees as required
  • Complete assigned tasks in a timely manner
  • Follow AltaPointe policies and procedures
  • Attend required in-service training and other workshops

Qualifications:

  • Bachelor's degree in a mental health discipline
  • Certification of case management and in-home intervention training by Alabama State Department of Mental Health within 6 months of employment
  • Valid verifiable driver's license and clear driving record
  • Knowledge of child/adolescent specialty, including psychiatric disorders, growth and development, developmental disabilities, behavior management, and cultural diversity

Physical Requirements:

  • Frequent standing, sitting, and walking
  • Occasional forward bending, squatting, and kneeling
  • Lifting up to 10 pounds occasionally
  • Strong grasp and pushing/pulling force up to 25 pounds occasionally
