Job Title: Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the West Annex Clinic, part of the Veterans Health Administration. As an LPN, you will play a vital role in providing direct patient care and working collaboratively with our healthcare team to deliver exceptional patient outcomes.

Job Summary:

The LPN will work in a dynamic and fast-paced environment, providing care to patients with a variety of physical and behavioral health needs. You will be responsible for administering medications, performing treatments, and documenting patient information in a timely and accurate manner. Additionally, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
  • Participate in daily PACT team huddles and meetings to discuss patient care and progress.
  • Assist with intake of patients, preparing them for provider visits, and maintaining clinic flow.
  • Complete pertinent clinical reminders and preventative health screenings.
  • Document all data collected and nursing care provided in the patient's electronic medical record.
  • Report all relevant patient information to the Clinic RN or provider.
  • Prioritize effectively and manage time productively in the clinical setting.
  • Work effectively with others and communicate in an appropriate, tactful, and courteous manner.
  • Attend clinic safety huddles daily.
  • Assist providers with exams/procedures and serve as a chaperone.
  • Perform treatments and procedures, such as electrocardiograms, vital signs, weights, blood glucose readings, bladder scans, ear flushes, and compression stockings, based on documented competency.
  • Track and administer required immunizations.
  • Administer ordered medications in accordance with Medical Center and Nursing Service policy and monitor for any side effects.
  • Participate in the management and follow-up of patient care needs and requests.
  • Initiate procedure preparations, adapting them to patients' physical and emotional needs before, during, and after procedures, observing and managing therapeutic or possible complications resulting from the procedure.
  • Knowledgeably initiate and perform nursing measures to prevent complications by recognizing deviations and/or changes in patients' conditions.
  • Recognize and respond/initiate emergency measures.
  • Respond promptly to emergency situations in the clinic, including but not limited to administering CPR.
  • Perform venipuncture and specimen collection and preparing specimens for storage and/or transport after specific training.
  • Demonstrate and apply the use of VA computer system efficiently and safely, adhering to facility and VA policies.
  • Participate in daily PACT team huddles and meetings.
  • Maintain positive and constructive professional working relationships with patients, families, medical staff, volunteers, and coworkers.
  • Additional clinic duties as assigned.
Work Schedule:

Monday - Friday, with possible evenings, weekends, and holidays.

Requirements:
  • United States citizenship.
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
  • Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Ability to maintain confidentiality and handle sensitive information.
  • Ability to adapt to changing situations and priorities.
