Job Title: Home Health Community Nurse Full Time

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Admission Nurse. This is a full-time opportunity to make a meaningful difference in the lives of our clients and their families.

Job Summary:

We are looking for a compassionate and dedicated Registered Nurse to provide high-quality home health care services to our clients in Chester Springs, Pennsylvania. As a Home Health Admission Nurse, you will be responsible for assessing clients' needs, developing personalized care plans, and providing ongoing support and education to clients and their families.

Responsibilities:
  • Assess clients' needs and develop personalized care plans
  • Provide ongoing support and education to clients and their families
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Monitor clients' conditions and report changes to Clinical or Client Services Manager
  • Document observations, interventions, and evaluations in a state-of-the-art touch pad tablet
Requirements:
  • Current license as a Registered Nurse in Pennsylvania
  • Minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred, but not required
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
