About the Role

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in a weekend home health position. As a member of our team, you will provide high-quality patient care and support to our adult and geriatric clients in their homes.

Responsibilities
  • Develop and implement individualized care plans in accordance with patient needs and goals.
  • Conduct home visits to clients in designated geographic territories, providing medical care and support as needed.
  • Administer medication, perform wound care, and provide other medical treatments as prescribed by physicians.
  • Monitor client conditions, reporting changes to Clinical or Client Services Manager.
  • Follow up with and execute doctors' orders, ensuring accurate and timely documentation.
  • Perform client assessments as necessary, utilizing a state-of-the-art touch pad tablet to document observations and interventions.
  • Coordinate case management and services provided, ensuring seamless care delivery.
Qualifications
  • A current license as a Registered Nurse in New Jersey.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
