Ideal candidates will have experience working with patients facing behavioral health challenges and will be adept at navigating local community resources and benefit applications.

This position requires mobility, involving travel to patients' residences, treatment facilities, community settings, and assigned clinics to foster meaningful connections.

Key Responsibilities:
  • Manage Transitions of Care for selected patients, including follow-ups after emergency room visits and hospital stays.
  • Provide triage support and consultation to team members, addressing complex cases as they arise.
  • Create comprehensive care plans utilizing the 5Ms Geriatric best practice framework.
  • Develop a holistic understanding of patient needs related to Social Determinants of Health.
  • Identify and address barriers to patient engagement with essential resources and support systems.
  • Educate patients on managing chronic health conditions and available behavioral health options.
  • Act as a liaison between patients and healthcare providers, facilitating navigation through both internal and external systems.
  • Initiate care planning and coordinate action steps for high-risk patients in collaboration with the interdisciplinary team.
  • Empower patients to achieve their health goals through self-determination.
  • Refer patients to necessary services and supports.
  • Lead Interdisciplinary Team Meetings as required.
  • Assess family dynamics and conduct family meetings when necessary.
  • Contribute to the development and delivery of team training materials.
  • Facilitate group psychoeducation and support sessions within the center.
  • Perform additional duties as required.
  • Participate in interdisciplinary reviews and coordination for complex patient cases.
  • Uphold patient confidentiality in compliance with HIPAA regulations.
  • Document patient interactions in the medical record system promptly.
  • Adhere to general policies regarding safety, infection control, and attendance.

Required Qualifications:
  • Active Registered Nurse (RN) license.
  • At least 4 years of experience in human services and navigating community resources.
Preferred Qualifications:
  • Familiarity with state Medicaid guidelines and application processes is preferred.
  • Experience with patients facing behavioral health issues and substance use disorders is advantageous.
  • Prior experience conducting home visits and knowledge of field safety practices is preferred.
