  • As a Physical Therapist (PT) at Community Care Center of Hondo - AA, you will be responsible for effectively screening, evaluating, and rehabilitating patients with physical functioning disorders and/or wound care. You will develop appropriate care plans following all regulatory and clinical practice standards. You will utilize standardized assessments and evidence-based practices to support clinical interventions. You will administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. You will provide wound care and adjust treatments according to patient progress. You will evaluate treatment outcomes for effectiveness and modify services as necessary. You will provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. You will communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans. You will provide comprehensive discharge summaries of services provided, adhering to regulatory and clinical requirements. You will document all required information in the electronic documentation system and maintain patient records according to company policies. You will accurately record treatment minutes for services provided, adhering to all regulatory and clinical requirements. You will participate in required meetings, including patient care conferences, utilization review meetings, rehabilitation meetings, etc. You will engage in facility and company required trainings, in-services, and conferences. You will obtain necessary DME or supplies per regulatory and clinical practice requirements. You will supervise and support physical therapist assistants, rehabilitation aides, and/or students. You will adhere to APTA and State Required practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist.
