Key Responsibilities- Represent Enhabit Home Health & Hospice in external transitional care activities, fostering strong relationships with patients, families, physicians, hospitals, and other healthcare professionals.
- Develop patient-centered transition plans that promote quality and efficiency, integrating clinical guidelines and protocols.
- Serve as a public awareness representative, educating patients and families about our services and promoting our brand.
- Graduate of an approved school of nursing with a valid state license.
- At least one year of experience in home care, with a strong understanding of federal, state, and local laws and regulatory guidelines governing hospice operations.
- Basic technology skills, including operation of a mobile device.
- Demonstrated experience with and knowledge of hospice care.
- Valid state driver's license and reliable transportation.
