About the Role

The Program Manager 2 Leader will oversee all aspects of two programs, ensuring seamless coordination of care and supports. This position requires strong collaboration skills, working closely with a team to achieve shared goals.

Key Responsibilities
  • Direct Care & Coordination of Services: Ensure the quality of support for adults with disabilities and/or brain injuries living in supervised residential settings, promoting growth, independence, and dignity.
  • Maintain ongoing professional communication with supervisor and stakeholders.
  • Meet with the Nurse Manager weekly to discuss program operations and individual progress.
  • Support individuals in developing person-centered ISP plans, monitor progress, and ensure goal achievement.
  • Organize activities and opportunities for meaningful engagement, fostering increased independence through natural supports and community membership.
  • Collaborate with nursing counterparts on medical aspects of service delivery.
  • Ensure individuals' nutritional needs are met, provide support with preparation and eating as necessary.
  • Promote physical health and wellness routines.
Additional Responsibilities
  • Assist individuals with personal care tasks, including toileting, showering/bathing, grooming, and mobility assistance.
  • Support individuals with Activities of Daily Living (ADL) tasks, such as cooking, cleaning, and general maintenance.
  • Ensure program coverage in accordance with funding source standards, seeking assistance as needed.
Quality Management
  • Promptly identify program areas of need and address them, seeking assistance as needed.
  • Maintain knowledge of ongoing program issues and developments, informing supervisor of program concerns.
  • Follow funder and agency protocol for reporting significant incidents and alleged abuse/neglect.
  • Adhere to submission deadlines for ISP-related documents and incident reports.
Requirements
  • Bachelor's Degree preferred or three years experience in a human service field required.
  • Valid driver's license in state of residence.
  • Two- to three-years supervisory experience required.
  • Basic computer literacy, including data recording, electronic time management, and email usage.
  • Effective critical thinking and communication skills.

Estimated salary: $60,000 - $80,000 per year, depending on location and experience.
