We provide compassionate primary and post-acute care to seniors in various settings, including assisted living, life plan communities, independent living, skilled nursing, and long-term care facilities across the United States.
Key Responsibilities
- Deliver provider-level direct patient care services within the post-acute care environment.
- Conduct clinical interventions and episodic care visits, along with appropriate follow-up evaluations.
- Manage ongoing evaluation and treatment of chronic illnesses as clinically indicated.
- Assess and treat residents experiencing changes in their health status.
- Review and manage medications, orders, and diagnostic tests, responding to clinical abnormalities as necessary.
- Collaborate with physicians for comprehensive medical management.
- Perform Annual Wellness Visits for long-term nursing facility residents and conduct Annual History and Physicals as needed.
- Assist with facility-wide health issues when appropriate collaborations are in place.
- Coordinate care with other healthcare professionals, including therapists, specialists, and social workers.
- Maintain open communication with families and participate in family conferences when possible.
- Fulfill additional duties as assigned.
- Valid nurse practitioner or physician assistant license in the state of practice & current board certification in your specialty.
- Master's Degree in Nursing or Physician Assistant Studies.
- Preferred strong leadership, interpersonal, and problem-solving skills.
- Excellent written and verbal communication abilities are essential.
- Must demonstrate punctuality, organizational skills, and the ability to work effectively within a team.
