Pharmacist Outpatient Specialist

As a Clinical Pharmacist with CHI St. Alexius Health, you will play a vital role in providing exceptional patient care and contributing to the success of our health network. Key Responsibilities
• Collaborate with healthcare teams to provide comprehensive pharmaceutical services, including drug distribution and clinical services.
• Monitor drug therapy and communicate findings and recommendations to appropriate professionals.
• Participate in renal dosing protocol and accurately assess medication orders for appropriateness.
• Actively work to help direct nursing staff in resolving problems and errors.
• Participate in registering and following Clozapine patients in IVAX.
• Be scheduled to cover various areas, including IS, OR, CP, IP, and Clinical.
• Participate in pharmacokinetic monitoring, anticoagulant management services, and pain management.
• Prepare and check intravenous admixtures, provide timely drug information, and document drug information questions.
• Actively participate in ADR reporting, medication error reporting, and make recommendations for improving safety in the medication use system.
• Develop formulary drug monographs, promote Formulary Management, and support Formulary Management decisions.
• Participate in writing, revising, or reviewing departmental/house-wide policies and procedures.
Requirements
• Graduate of a College of Pharmacy with a minimum of a Baccalaureate Degree in Pharmacy.
• Licensed as a pharmacist in the State of North Dakota in good standing.
