Key Responsibilities:

  • Facilitate discharge planning and length of stay oversight in collaboration with the healthcare team.
  • Intervene with patients who have complex or high-risk psychosocial needs.
  • Perform duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.

Requirements:

  • Graduate of an accredited School of Nursing, required.
  • Current License in the state of employment, required.
  • 2-3 years acute care hospital setting, preferred.
  • Care Management or Utilization Management experience, preferred.
