The Physical Therapist plays a crucial role within a multidisciplinary team at Centrix Health Resources. The primary objective of home health physical therapists is to assist patients in regaining and maintaining their independence within their home environment. The role is centered on restoring patient functionality, alleviating discomfort, and preventing disabilities through the implementation of medically prescribed physical therapy.
Key Responsibilities:
- Collaborate with physicians to assess the patient's functional capabilities.
- Evaluate the home setting for safety concerns and identify necessary equipment.
- Identify functional challenges and develop a tailored care plan aimed at enhancing the patient's functional abilities, making adjustments as needed.
- Conduct various patient assessments, including range of motion tests, manual muscle evaluations, gait analysis, and record findings to refine treatment strategies.
- Perform initial and ongoing assessments along with the required documentation in line with agency protocols.
- Regularly review and update the patient's therapy program to optimize functional recovery.
- Implement exercise regimens using specialized therapeutic techniques and equipment.
- Design and deliver prescribed physical therapy interventions to restore function, alleviate pain, and avert disability following medical conditions, injuries, or loss of body parts.
- Advise on prosthetic and orthotic devices, including minor adjustments or recommendations for modifications.
- Engage with family members, referring physicians, and other healthcare professionals to ensure coordinated and high-quality patient care.
- Complete and submit all necessary documentation, including evaluations, care plans, clinical notes, and progress reports, in a timely manner.
- Ensure all patient visit notes are submitted by the next scheduled workday, not exceeding forty-eight (48) hours.

Qualifications:
Applicants must have graduated from an accredited Physical Therapy program and hold a valid license in the state of Arizona.
