Key Responsibilities:
- Delivering and coordinating nursing care in alignment with the Hennepin Healthcare System's Relationship-Based Care model.
- Conducting patient assessments to determine the severity of symptoms and directing them to the appropriate level of care.
- Providing education to patients and their families regarding health management and disease processes.
- Documenting nursing interventions and developing individualized care plans.
- Ensuring continuity of care and promoting preventive health measures.
Telehealth Nursing Role:
The Telehealth Staff Nurse is responsible for providing remote nursing care through telecommunications technology, ensuring patients receive the necessary support 24/7. This role involves:
- Utilizing evidence-based protocols to assess patient needs and recommend appropriate care.
- Managing medication refill requests and collaborating with pharmacies to facilitate patient care.
- Adhering to established policies and procedures while maintaining professional nursing standards.
Qualifications:
Minimum Requirements:
- Graduation from an accredited nursing program.
- Valid Registered Nurse license in the State of Minnesota.
- At least one year of professional nursing experience.
- Experience in an ambulatory care setting within the last three years.
- Familiarity with EPIC Inbasket for patient communication.
Preferred Qualifications:
- Certification in a clinical specialty area.
- Bachelor of Science in Nursing (BSN) is preferred, with encouragement for ongoing education and professional development.
