Compensation Package:

  • We offer a highly competitive salary range of $120,000 - $180,000 per year, depending on experience and qualifications.

About the Role:

Job Summary:

The Advanced Healthcare Practitioner will be responsible for providing high-quality, patient-centered care to seniors in our primary care settings. This role requires strong clinical skills, excellent communication and interpersonal abilities, and a commitment to delivering exceptional patient outcomes.

Key Responsibilities:

  1. Assess patient needs and develop personalized care plans to address their unique health challenges.
  2. Provide routine check-ups, diagnostic testing, and treatment as needed, using evidence-based medicine principles.
  3. Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between providers.
  4. Foster strong relationships with patients and families through empathetic listening, clear communication, and proactive problem-solving.
  5. Stay up-to-date with the latest clinical guidelines, research findings, and best practices in primary care, applying this knowledge to improve patient outcomes and enhance practice performance.

Requirements:

  • Masters or Doctoral degree in Nursing or related field.
  • Current certification as an Advanced Practice Registered Nurse (APRN) in the state of practice.
  • Board certification by AANP or ANCC preferred but may be required for certain states.
  • Minimum 1 year of experience in primary care or related field.
  • Excellent clinical skills, with ability to work autonomously and collaboratively within interdisciplinary teams.
