LPN/RN Charge Nurse Position

Athena Health Care Systems is seeking a skilled LPN/RN Charge Nurse to join our team at Valerie Manor in Torrington, CT.
Job Summary:The LPN/RN Charge Nurse will provide direct nursing care to residents and supervise the day-to-day nursing activities performed by the nursing assistant. This role requires a strong understanding of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to long-term care.
Responsibilities:- Provide direct nursing care to residents, including administering medications, performing treatments, and documenting resident records.
- Supervise the nursing assistant and ensure that they are performing their assignments/duties in accordance with policies and procedures and standards of practice.
- Review resident records daily to assure accuracy and completeness, and maintain accurate awareness of residents on alert list.
- Document comprehensive and complete nursing notes, and communicate relevant clinical information to CNA's, Nursing Supervisor & oncoming shift.
- Transcribe and follow physician's orders promptly and accurately, and render and/or assist with completing nursing care, treatments and medication administration, dressings, and other physician orders as applicable.
- Participate in resident care plan meetings, and maintain and oversee current Resident Care Plans and CNA Care Cards on all residents under his/her care.
- Follow Pharmacy policies and procedures, and maintain an accurate and safe control of all drugs for which he/she is responsible.
- Participate in the on-call program for the nursing department as directed by the DNS, and ensure that CNA's follow established Infection Control Procedures to prevent the spreading of infection.
- Complete Accidents and Incidents reports (Reportable events), ensuring that all appropriate notifications, documentation, investigation, reporting, and interventions are complete.
- Initiate any and all emergency/disaster procedures as necessary, and be familiar with and ensure that staff adheres to safety, health, fire, disaster, emergency, sanitation codes and MSDS.
- Complete yearly performance evaluations of CNA's as directed by the DNS, and respect and take care of facility equipment and is not wasteful of supplies.
- Participate in surveys and inspections by government agencies and implement plans of correction for deficiencies as directed by the DNS.
- Monitor and review all at risk residents (I+O's, weights, restraints, falls, wounds, etc.), and oversee CWC program and ambulation programs as directed.
- Participate in committees and meetings, and provide input for Quality Improvement in the Nursing department.
- Assist Director of Nurses with disciplinary process when assigned, and comply with and follow facility Privacy Practices, policies & procedures and protects residents' individual health information.
- Continues growth and expands job knowledge, and other duties deemed appropriate and assigned by your supervisor.
