Job Title: Licensed Practical Nurse

Northpoint Recovery Holdings, LLC is a growing behavioral healthcare treatment platform treating adults with substance use disorder and co-occurring conditions. We are seeking a Licensed Practical Nurse (LPN) to join our team in a full-time capacity.

Job Summary:

The LPN will provide and oversee medical services to a caseload of patients, acting as the medical treatment plan coordinator for the multi-disciplinary team. This role requires strong communication and interpersonal skills, with the ability to work with a diverse population.

Responsibilities:
  • Interact with patients, observe behaviors, and communicate significant observations to the treatment team
  • Administer medications and engage in education process
  • Utilize de-escalation skills to maintain personal safety and patient well-being
  • Provide admission, orientation, education, and discharge plans to patients and families
  • Consult with the treatment team in the development of treatment plans and discharge planning
  • Participate in maintaining a clean, orderly, and safe environment
  • Admit and discharge patients
  • Obtain vital signs, monitor symptoms, and document in the patient record
  • Comply with laws, regulations, and scope of licensure
  • Monitor patients to assess needs and facilitate medical treatment and interventions
  • Document medical assessment, diagnosis, progress notes, rounds, and treatment planning items
  • Actively participate as a member of the treatment team and maintain effective working relationships
  • Maintain professional boundaries with patients
  • Oversee quality assurance and compliance activities
  • Provide medical updates to utilization review and managed care representatives
Qualifications:
  • Current, unencumbered, active license to practice as a Licensed Practical Nurse in the state where services are rendered
  • Must be at least 21 years of age
  • Current and nationally accredited CPR/First Aid/AED certification
  • Knowledge of computer and electronic medical records systems (KIPU)
  • Enthusiasm, passion for working with people, and internal drive to improve lives
  • Two years of experience as a Registered Nurse with one year inpatient/residential experience preferred

Conditions of Employment:
  • Completion of a tuberculin screening during first week of employment
  • Completion of a pre-employment drug screening, post-accident, and upon reasonable suspicion of use
  • Completion of background and professional reference checks
  • Completion of a Motor Vehicle Record check
