Licensed Practical Nurse

Northpoint Recovery Holdings, LLC is a growing behavioral healthcare treatment platform treating adults with substance use disorder and co-occurring conditions. We are seeking a Licensed Practical Nurse (LPN) to join our team in a full-time capacity.
Job Summary:The LPN will provide and oversee medical services to a caseload of patients, acting as the medical treatment plan coordinator for the multi-disciplinary team. This role requires strong communication and interpersonal skills, with the ability to work with a diverse population.
Responsibilities:- Interact with patients, observe behaviors, and communicate significant observations to the treatment team
- Administer medications and engage in education process
- Utilize de-escalation skills to maintain personal safety and patient well-being
- Provide admission, orientation, education, and discharge plans to patients and families
- Consult with the treatment team in the development of treatment plans and discharge planning
- Participate in maintaining a clean, orderly, and safe environment
- Admit and discharge patients
- Obtain vital signs, monitor symptoms, and document in the patient record
- Comply with laws, regulations, and scope of licensure
- Monitor patients to assess needs and facilitate medical treatment and interventions
- Document medical assessment, diagnosis, progress notes, rounds, and treatment planning items
- Actively participate as a member of the treatment team and maintain effective working relationships
- Maintain professional boundaries with patients
- Oversee quality assurance and compliance activities
- Provide medical updates to utilization review and managed care representatives
- Current, unencumbered, active license to practice as a Licensed Practical Nurse in the state where services are rendered
- Must be at least 21 years of age
- Current and nationally accredited CPR/First Aid/AED certification
- Knowledge of computer and electronic medical records systems (KIPU)
- Enthusiasm, passion for working with people, and internal drive to improve lives
- Two years of experience as a Registered Nurse with one year inpatient/residential experience preferred
- Excellent organizational abilities
- Excellent written and oral communication skills
- Attention to detail and accuracy
- Patient service-oriented
- Creative and persistent problem solver
- Able to handle confidential material
- Ability to interact and communicate with individuals at all levels of organization
- Strong interpersonal skills
- Ability to multi-task and prioritize workload
- Completion of a tuberculin screening during first week of employment
- Completion of a pre-employment drug screening, post-accident, and upon reasonable suspicion of use
- Completion of background and professional reference checks
- Completion of a Motor Vehicle Record check
