Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Assured Home Health, a part of LHC Group. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improve their quality of life.
Responsibilities:- Evaluate patients' functional status and physical therapy needs
- Assist physicians in developing therapy plans of care
- Provide services within the scope of practice as defined by state laws
- Observe and report patients' responses to treatment and any changes in their condition
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning
- Instruct and inform healthcare teams, patients, and caregivers regarding plans of care, functional limitations, and progress toward goals
- Current Physical Therapy licensure in the state of practice
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
