Cedar Lake Assisted Living & Memory Care is seeking a skilled Wellness Nurse LPN to join our team. As a Wellness Nurse LPN, you will play a vital role in supporting the health and well-being of our residents.
Key Responsibilities:- Assist in the coordination of resident admission processes, including screening, assessment, and care needs.
- Ensure care is delivered as indicated in resident care plans.
- Administer medications and treatments as prescribed by physicians.
- Coordinate care and services with outside providers, including resident physicians.
- Document and report care provided and resident observations.
- Maintain a working knowledge of state regulations, policies, and procedures.
- Graduate from an accredited school of practical nursing.
- Current nursing license and state registration (LPN or RN).
- Three years' experience in assessment and geriatric nursing, preferred.
- Eligibility for medical insurance, dental insurance, vision insurance, life insurance, and other voluntary insurance options.
- Participation in our 401(k) plan.
