We are seeking a highly skilled Registered Nurse to join our team as a Nurse Navigator for Pediatric Cardiology at St. Luke's Health System. As a key member of our healthcare team, you will play a vital role in providing exceptional patient-centered care to children with heart health conditions.
Responsibilities- Collaborate with Pediatric Cardiology specialists to coordinate patient care and ensure seamless transitions between healthcare services.
- Work closely with the Care Coordinator to obtain, organize, and send relevant patient information, as well as assist with scheduling surgical implants.
- Develop and maintain strong relationships with multiple health organizations and their staff to ensure effective communication and coordination of care.
- Bachelor's degree in Nursing
- At least 2 years of RN experience
- Current RN license in the State of Practice and Basic Life Support (BLS) certification
