Position Overview: Partners In Care, Inc. is in search of a Certified Nursing Assistant to provide exceptional care to our hospice patients within our esteemed in-patient facility. This role involves working closely with a diverse team of healthcare professionals, including registered nurses, social workers, chaplains, and volunteers, to improve the quality of life for our patients and their families.
Compensation: The salary range for this position is $21.00 to $28.00, depending on experience.
Sign-on Bonus: A bonus of $1,000 will be awarded after 90 days of employment.
Work Schedule: The position requires working three 12-hour shifts per week, from 6:30 am to 6:30 pm.

Key Responsibilities:
- Maintain professional communication with patients, families, and caregivers while ensuring confidentiality of health information.
- Evaluate patients' ability to undergo treatment prior to care initiation and reassess their condition post-care.
- Deliver personal care in accordance with the care plan, ensuring patient privacy and comfort.
- Utilize proper body mechanics and equipment to meet safety needs for both patients and oneself.
- Collaborate with care managers or registered nurses to keep care plans current and document any changes in patient conditions.
- Follow written and verbal instructions from the Registered Nurse on duty regarding patient care.
- Assist patients with personal care, mobility, and positioning, while guiding families or caregivers in personal care practices.
- Prepare and serve meals in accordance with established policies, assisting with feeding as necessary, and perform essential housekeeping tasks to maintain a safe environment.
Qualifications:
- Current CNA License.
- Ability to successfully pass background checks, drug screenings, and TB tests.
- Graduation from an accredited nursing program.
- Preferred experience of 1-2 years in a medical care setting, with a focus on home health or hospice care.
- Valid automobile insurance, an Oregon driver's license, and reliable transportation.
- Current CPR Certification.
- Strong communication and interpersonal skills.
- Capability to work both independently and as part of an interdisciplinary team.
- Demonstrated compassion, empathy, and commitment to patient care.
