Job Description:

Key Responsibilities
  • Collaborate with the interdisciplinary team to develop, implement, and evaluate individualized care plans for residents.
  • Document resident conditions, nursing needs, and treatment outcomes accurately and in a timely manner.
  • Lead and direct nursing assistant assignments, adjusting as needed based on census and level of care required.
  • Administer medications, treatments, and therapies as prescribed, documenting reactions and outcomes.
  • Communicate effectively with families regarding changes in medications, treatments, or resident conditions.
  • Establish and maintain accurate and up-to-date care plans, reflecting the unique needs and preferences of each resident.
  • Coordinate admissions, discharges, and transfers to ensure seamless transitions and high-quality customer service.
Requirements
  • Graduate of an accredited Licensed Practical Nurse program.
  • Current LPN licensure and certifications as required by state regulations.
