Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at BronxCare Health System. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our Primary Care Initiative.

Key Responsibilities
  • Under the supervision of a physician or Registered Nurse, you will apply the nursing process to an assigned group of patients, ensuring their safety and well-being.
  • You will interact professionally and courteously with patients, visitors, and other employees, maintaining a positive public perception of BronxCare Health System.
  • As a key member of our team, you will contribute to Performance/Quality Improvement activities, participating in data collection, analysis, and implementation of risk management and claims activities.
  • You will prepare patients for provider visits, including intake screening, pre-visit planning, and review of medical records.
  • Additionally, you will manage the flow of patients, monitor provider progress, and identify the next patient scheduled for a room.
Requirements
  • Graduate of an accredited LPN program
  • Active NYS LPN licensure
  • Current BLS certification
  • High School or GED
  • Knowledge of EMR systems/programs
  • Microsoft Office Suite
