Job Responsibilities:
- Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse
- Develop and implement individualized care plans, in collaboration with the patient's physician and healthcare team
- Conduct thorough assessments, monitor patient progress, and report changes to the supervisor and/or physician as necessary
- Document all patient interactions, including assessments, nursing actions, and patient responses
Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
Preferred Qualifications:
- Six months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
Physical Requirements:
- Ability to speak, write, read, and understand English
- Ability to travel, lift 50 pounds, and move equipment without assistance
- Prolonged walking, standing, bending, kneeling, reaching, twisting
