Concho Hearts Hospice is seeking a dedicated and compassionate caregiver to join our team. As a Hospice Part Time CNA, you will provide high-quality care to patients in their homes, focusing on symptom management and patient and family support.
Responsibilities:- Administer prescribed treatments and medications to ensure patient comfort and well-being.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Maintain accurate and up-to-date medical records, ensuring confidentiality and compliance with regulations.
- High School Diploma or equivalent required.
- Current driver's license and reliable transportation.
- At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, or hospice setting.
- Ability to read, write, and verbally communicate effectively with patients and staff.
- Mileage pay for travel to patient visits.
